A standard sick certificate may be insufficient where an institution form, real-time consultation, functional-impact assessment or supporting proof is required.

What evidence may be accepted?

UTS prefers its Professional Authority form. It may also consider a hospital certificate or a sufficiently detailed medical certificate from a registered health practitioner. Pharmacist certificates are not accepted for this process.

Certificate and form requirements

Required form

The Professional Authority form is preferred, not exclusive. It asks for AHPRA registration and Medicare provider numbers, practitioner details, signature, consultation and impact dates, and severity.

Face-to-face, video-conference and telephone consultations are explicitly accepted.

Previous dates

No general backdating rule was identified. Evidence must be current and state issue, consultation and impact dates. Events an unreasonable length in the past are ineligible.

Duration

Different deadlines and adjustment limits apply to examinations, faculty assessments, long extensions, placements and mandatory attendance.

Impact wording

Evidence should state the severity, effect on the student's ability to complete the assessment or examination, affected duration and the practitioner's contact and registration details.

Submission deadline

Deadlines vary: some examination applications are due within 48 hours, faculty assessments within two working days, long extensions before the due date, and placement applications within five working days.
