What evidence may be accepted?
UQ may accept evidence from specified registered Australian health professionals. Practitioner eligibility and the permitted scope vary by assessment type; evidence must be independent and signed.
Certificate and form requirements
Required form
No prescribed UQ practitioner form was identified for the general process. A signed certificate or statement is uploaded through the relevant student application system.
Consultation modality
Current university-wide guidance does not expressly approve or prohibit telehealth. Faculty-specific guidance must not be generalised to all UQ students.
Previous dates
Evidence should normally be obtained on or before an assignment due date, or within two business days after an examination. Later evidence requires an explanation of why earlier access was impossible or unsafe.
Duration
An extension cannot exceed the documented affected period and is generally capped at 28 days in total.
Impact wording
Evidence should explain how the condition affected the student's capacity to submit the assignment or undertake the examination. Illness arising during an examination requires more detailed evidence than a generic fitness statement.
Submission deadline
Extension applications are generally timely when received within 24 hours after the due date. Deferred-examination applications are generally due within five calendar days after the original examination.
