Certificate and form requirements
Required form
Health Professional Report v4.7 is preferred but not mandatory. It asks a practitioner to assess functional impact across academic tasks, administrative tasks and attendance.
Consultation modality
A consultation is required and may occur in person or by telehealth. A certificate issued without a consultation is not accepted.
Previous dates
No general rule expressly approving or prohibiting backdating was found. If the report is completed more than four business days after the due date, the practitioner must explain why the student could not apply on time.
Duration
The Health Professional Report is intended for short-term impacts of up to six weeks. Longer or ongoing circumstances may require other support arrangements.
Impact wording
The practitioner should explain the effect on preparing for, attending or completing assessment and classify the functional impact as moderate or severe where the form requires it.
Submission deadline
Apply no later than four business days after the assessment due date or missed class. Supporting documents may generally be added within four business days of applying.
