What evidence may be accepted?
QUT may consider a certificate or letter from a registered health practitioner, including specified online doctors, pharmacists and registered nurses, when the evidence meets the concession requirements.
Certificate and form requirements
Required form
No QUT practitioner form is required for ordinary academic concessions. The separate Health Professional Assessment Form is for withdrawal without penalty and should not be presented as the standard concession form.
Consultation modality
Telehealth and online doctors are explicitly included in current QUT evidence guidance.
Previous dates
Retrospective evidence may be considered only when the practitioner gives a clinical opinion that the student was unfit on the stated earlier dates.
Duration
Assignment extensions normally do not exceed 14 days and should match the documented affected period.
Impact wording
Evidence should identify the relevant assessment or examination dates, nature or severity where appropriate, and the practitioner's opinion about the student's ability to study, complete the task or sit the examination.
Submission deadline
Extension requests should be made before the 48-hour late-submission period ends. Special consideration and deferred-examination applications are normally due within three working days of the assessment or examination.
