What evidence may be accepted?
Medical certificates and practitioner letters may be used when they come from an appropriately registered health professional whose expertise is relevant to the circumstances. Australian evidence should generally identify the practitioner, registration and contact details.
Certificate and form requirements
Required form
No mandatory Monash practitioner form was identified. The certificate or letter should be on practitioner letterhead, signed and dated, and include the relevant provider details.
Consultation modality
A genuine consultation is required. For many assessment types Monash expects an in-person consultation or real-time phone or video consultation where attending in person was impractical. An asynchronous online certificate is normally insufficient.
Previous dates
Retrospective evidence needs a clinical explanation of why it was issued later and how the practitioner reached an opinion about the earlier affected period. Acceptance is not automatic.
Duration
Ordinary assessment extensions are generally limited to 10 calendar days and must be proportionate to the documented impact. Longer disruption may need a different academic arrangement.
Impact wording
Evidence should give the practitioner's opinion about the student's ability to study or undertake the particular assessment, with exact affected dates. Attendance-only notes or statements that simply repeat the student's account may be insufficient.
Submission deadline
Apply as soon as possible and no later than 11:55 pm in the unit's time zone on the assessment due date. Missing evidence may usually be added within three calendar days when the delay is explained.
